ACMH Poised to Roll Out Care Coordination Services for Medicaid Health Home Recipients
ACMH is scheduled to convert its stand-alone Case Management Services to a Care Coordination Model in early 2012. Under the NYS Health Home initiative, Medicaid members with chronic illnesses will benefit from more fully integrated and coordinated care. ACMH has a proven track record of engaging consumers and facilitating care coordination across physical and behavioral health systems.
74 Avenue A Community Residence Opens in Queens Village
With capital dollars from the New York State Office of Mental Health (OMH) and Low Income Housing Tax Credit (LIHTC) equity investment, ACMH completed the conversion of a portion of a former State hospital building on the grounds of Creedmoor Psychiatric Center into a 55 bed transitional community residence.
74 Avenue A, featuring a combination of shared suites, studio apartments, and a Community Center admitted its first residents in January 2012. A grand opening will take place on April 20.
Markus Gardens Breaks Ground
Now under construction in Jamaica, Queens, with a scheduled December 2013 opening, this apartment building will provide 60 studio apartments for adults and shared two-bedroom apartments for 16 young adults, ages 18-25, with enhanced services to promote positive youth development and transition to independence.
Capital funding is a combination of State grants, Low Income Housing Tax Credit (LIHTC) equity, and seed money from private donors, including a major bequest from the Estate of Rita Markus.
ACMH Awarded Funding for Affordable Housing Project on E. 144th Street, Bronx
Governor Cuomo recently announced an LIHTC award to ACMH under the State?s new Consolidated Funding Application process that promotes regional economic development. With additional funding from OMH under the NY/NY III initiative, this project will provide studio apartments for 22 adults and 20 young adults with mental illness and 18 studio, one-bedroom and two-bedroom units for low-income community singles and families.
An early-stage development grant from the Deutsche Bank Americas Foundation and an acquisition loan from the Corporation for Supportive Housing have made this project possible.
ACMH Supported Housing Opportunities
With funding from OMH, Supported Housing provides a rental stipend, as well as support services from mental health professionals to assist persons who have been diagnosed with a serious mental illness to establish and maintain their own permanent household. With an additional 9 beds awarded in the 2011/2012 Fiscal Year, ACMH now offers 102 Scattered-Site Supported Housing opportunities in Manhattan, Queens, and the Bronx.
